Sustainable Tailored Integrated Care for Older People in Europe – SUSTAIN Final Conference

The EU-funded project SUSTAIN, in which AGE has been involved, aims to take a step forward in the development of integrated care for older people. The conference will present the different aspects of the project: the research carried out within its framework, the integrated care sites which took part in the project, and the achieved results, including the SUSTAIN Roadmap for Integrated Care.
