Connecting different therapeutic areas and specialties, integrated care focuses on the needs of the recipient of care and on the coordination between diagnosis and treatment and between primary and secondary care. Integrated care helps improve the efficiency of diagnosis and treatment, and facilitate prevention.
However, the complexity of health and care systems poses great challenges. The EU-funded SHAPES project, in which AGE is involved, aims to provide guidelines, a roadmap and an action plan to foster the large-scale deployment and adoption of digital solutions and new integrated care services in Europe.
Intended to representatives from industry, academics, health and care, civil societies as well as older people, this 2nd dialogue workshop will unveil the project’s preliminary findings.
