The ValueCare experience: Creating value for older people with chronic conditions in Europe

The book The ValueCare Experience: Creating Value for Older People with Chronic Conditions in Europe—A Transition Towards Outcome-Based Integrated Care Supported by Technology” is an output of the ValueCare project. It provides insights into the project’s findings and offers a roadmap for implementing value-based, integrated care models across Europe.

The project, in which AGE has been involved, has developed a Value-Based Healthcare (VBHC) model that focuses on providing the highest possible value to older people in need of care. It aims to shift away from volume-based care models to efficient delivery of outcomes that truly matter to older people.

This publication aims to guide healthcare managers, practitioners and policymakers by providing practical guidance and strategies to implement VBHC in Europe.
By sharing real-world examples and lessons from the seven pilots of the project, the book demonstrates how VBHC can be effectively integrated into existing systems across Europe.

Each chapter offers a detailed exploration of the various components that constitute the ValueCare approach, including the role of technology. It also addresses the challenges encountered along the way, offering valuable insights into the practicalities of transforming health care delivery.

As a project partner and consortium member, AGE has significantly contributed to this publication, particularly, chapter 9 on policy.
